CHARGES AND INSURANCE INFORMATION

Treatment

Prior to receiving treatment, patients will complete a brief Medical History questionnaire and will be screened to determine if they are suitable candidates for this procedure. The treatment cost is $429  which covers all necessary testing including EKG, Lung Function Test, examinations and medications administered during your visit. Patients will also be provided with the necessary take-home prescription medications. Payment is due at the time of treatment.

Insurance

Patients will be given appropriate documentation to submit to their insurance company for potential reimbursement. FREELIFE Stop Smoking Clinics does not accept Assignment of Insurance and makes no claim as to reimbursement for treatment charges. This procedure is not currently covered by Medicare or Medicaid.

Tax Deduction

The Internal Revenue Service (IRS) has ruled that smoking cessation programs administered by licensed medical physicians are tax deductible up to $350. Please contact your tax advisor or the IRS for additional information. FREELIFE Stop Smoking Clinics makes no claims as to tax deductibility of our treatment charges.
